MediSave for seniors, explained plainly.

What Flexi-MediSave actually covers for a senior's chronic conditions and outpatient care, the higher limits taking effect from 1 January 2027, and what to do once the yearly limit runs out — for families, caregivers and Active Ageing Centre managers fielding the question.

Flexi-MediSave lets seniors use their own or a family member's MediSave savings for chronic-condition and outpatient treatment, on top of subsidies and insurance. Right now the yearly limit is $500 for simple chronic conditions and $700 for complex ones, with an extra $400 a year for seniors aged 60 and above. From 1 January 2027 those limits rise to $700 and $1,000.

None of this replaces MediShield Life or subsidies — it's the layer that comes after them, for the ongoing outpatient costs that add up over a year of managing a chronic condition.

How much can a senior claim right now?

Flexi-MediSave withdrawal limits are set per patient, per year, and apply on top of any subsidies and insurance payouts:

What changes on 1 January 2027

MOH's reply confirms the raised limits are already set, not merely proposed: simple chronic conditions move from $500 to $700 a year, and complex chronic conditions move from $700 to $1,000 a year. MOH expects the change to benefit more than 900,000 patients.

Why it matters: if a senior is already close to this year's limit, the increase doesn't help retroactively — but it's worth timing an elective or non-urgent outpatient claim for January 2027 rather than December 2026 where the choice is genuinely open, since the higher limit resets with the new year.

What if a senior has already used up their MediSave limit?

The order is: subsidies and insurance apply first, MediSave (up to the yearly limit) covers what's left. If a patient still faces real difficulty after all three, MOH's position — stated in a separate Parliamentary reply on healthcare affordability — is direct: "no Singaporean will lose access to appropriate healthcare due to an inability to pay." The safety net beyond MediSave is MediShield Life and MediFund, and the practical route to it is a medical social worker at the treating hospital or polyclinic — not a form or a hotline.

For AAC managers: this is worth putting in plain terms at a caregiver briefing. Skipped follow-ups and unfilled prescriptions are the kind of thing an older person rarely volunteers, and the fastest way to unblock a stuck bill is to ask the treating institution for its medical social worker directly, rather than waiting for the senior to raise it.

Does MediSave cover palliative and hospice care?

Yes, on the inpatient side: inpatient palliative care is already covered by MediShield Life and all Integrated Shield Plans. For home palliative and hospice services, MOH's reply on palliative care access (19 August 2026) states that patients generally do not need to pay any cash out-of-pocket after subsidies, MediSave and the charity funding service providers receive. MOH also points families toward the Legacy Planning Campaign and Advance Care Planning (ACP), run with the Singapore Hospice Council, to have these conversations before a crisis rather than during one.
